Replace `unsigned int` by `std::size_t`

IN most case, the unsigned int were used one way or another as container indexes, and cast were therefore involved.

It proved a pain when dealing with tickets such as #1640 (closed) as introducing a strong type generated cast issues (I generally fixed the unsigned int -> std::size_t along with the change).
