Standalone OCR models

• List models independently from documents
• Allow the creation of a standalone model from separate form (/models/new/)
• Models are automatically linked to documents once they have processed one of its parts

I don't see a way to actually bind a model to a document to make use of it or am I missing something? I need to fix some migration not being reversible before I can properly test it though.
