- Add layer manager responsible for initializing all the concepts of a
  layer.
- Add plugin manager recorder responsible for adding plugin manager of
  each concept to the layer manager hash table.
- Modify macros DTK_DEFINE_CONCEPT, DTK_DECLARE_CONCEPT to take into
  account new concept management.
- Add macro DTK_DEFINE_LAYER_MANAGER to add easily a layer manager into a layer.
